Online Appointment Request

An appointment request can be made online. We will confirm your appointment by e-mail or telephone. Please send your request at least 48 hours before your desired appointment time.

When scheduling online, please also print and complete your Patient Registration Forms to bring with you to your appointment. This will save you time in the waiting room, and you can be seen more quickly! Click here to access our Patient Registration Information.

Availability

Appointments with Middlesex Orthopedic Surgeons physicians are timely. If the requested physician's availability is limited due to scheduling conflicts and/or specialty limitations, our staff can schedule an appointment with another Middlesex Orthopedic Surgeons physician. Patients may schedule appointments without a physician's referral if their insurance carrier allows.

We ask that all patients arrive at the clinic prior to their scheduled appointments to allow time to register properly, provide a medical history and complete the radiology X-rays process.
